 What is Social Prescribing?

Taking care of your health and wellbeing involves more than just medicine. You can improve your health and wellbeing through Social Prescription.

Social prescribing links patients/local people with health conditions to non-medical support to improve their health and wellbeing.

Social Prescribing at CWTC

Our aim to support people in improving your health and social welfare, make a great and good life choice through Social Prescribing at CWTC.

At CWTC, we provide:

  • activities to patients/local people connecting them to our creative arts activities exercise
  • wellbeing café
  • health awareness sessions
  • information and advice through health and social services
  • volunteering opportunities helping to reduce social isolation and improving mental and physical health.

By engaging in the activities at CWTC, the benefits are:

Simply talk to your GP, surgery staff, medical centres or fill in our self-referral form or social prescribing form at your surgery or online at:
